Gerry has extensive education and experience in Geriatrics and Home Care.  Gerry received her nursing degree from St. Luke’s Hospital Nursing School and holds a Bachelors in Health Administration.  Gerry is a Certified Dementia Practitioner,  Certified Patient Advocate and Geriatric Care Manager.  She consults with families to assist with Long Term Care Planning for loved ones.  Gerry provides education programs in our local communities and faciliates our Caregiver Support Groups.  She is a Master Trainer with the Matter of Balance Program which teaches individuals how to improve strength, balance and safely live at home.

Eileen Freitag is a Partner and Director of Fazzi’s Strategic Consulting Division. She is the past President and CEO of one of the largest home care agencies in the United States at that time, the Boston VNA; has served as the CEO of a long term care continuum that included a major home health component; and the Vice President of Business Development of a start-up software company. During her 40 year career in home care, she has served on the boards of several local and national health care organizations. Eileen’s division works with home health agencies on a variety of marketing, customer service, referral enhancement and interim management efforts.

Melinda and Mark Cannon founded the company in April 2001 to provide financial, reimbursement, clinical and cost reporting services to the home health, hospice and rehabilitation therapy industries. Melinda’s priority remains bridging the gap between clinical and financial issues in home health agencies. She is a speaker for multiple state home care associations and serves on the NAHC-HHFMA Work Group and is ex-officio on the Advisory Board. She  is also the author of The Home Health Pocket Guide to OASIS-C1.

Dan has been with the Medicare program since 2004. In 2008, he became part of Palmetto GBA’s Congressional Unit. This unit handled complex inquiries for Part A, Part B and Home Health and Hospice. His experience is extensive in, but not limited to, coverage polices, compliance, billing, enrollment, and systems. In 2007 and 2008, he was a manager for Palmetto GBA’s Beneficiary Call Center. Prior to working with Medicare, Dan worked in skilled nursing facilities for 12 years. He directed Dietary Departments, which included participation in nutritional assessments, care planning, wound rounds, and patient documentation.

J’non Griffin is a 30 year veteran of home care as an RN. She received her Master’s Degree in Health Care Administration in 2005.  She has experience as a field nurse, director and executive with home health and hospice agencies, both large and small. She has served as Director of Staff Development and Appeals for home health and hospice agencies. She has taken part in mock surveys for agencies, and prepared agencies for accreditation. She has been directly involved in accreditation surveys, acquisitions and many regulatory crises with state survey agencies and the intermediary.

David’s understanding of the goals of his clients is augmented by his clinical healthcare experience, which includes earning a medical degree, briefly practicing clinical medicine, and conducting award winning basic science research. He earned a Bachelors degree in biology at Harvard College, and an M.D. at the Columbia University College of Physicians and Surgeons.

Elizabeth is an attorney in private practice with extensive experience in health care. Prior to becoming an attorney, she was employed as a personnel manager by a Blue Cross-Blue Shield Plan. She studied at the University of Maryland School of Law, concentrating in health law, and gained considerable clinical experience, course work and employment experience in this area. Following her admission to the Bar, she developed an active practice in health law. She represents clients all over the country. Her clients are professional associations, managed care providers, and institutional health care providers, including hospitals, long-term care facilities, home health agencies, durable medical equipment companies, and hospices. She has also represented a number of individual providers and case managers concerning health-related issues. Ms. Hogue gives approximately 50 presentations each year on health-related topics, including future trends in the healthcare industry. Her numerous articles are regularly included in trade association publications and other journals. Ms. Hogue is a member of the American Health Lawyers Association. Steve previously was Chief Operating Officer with Evangelical Homes of Michigan in Farmington, Michigan.  Steve joined Evangelical Homes of Michigan as its Corporate Director of Wellness and Physical Health in 2007 after owning and operating several wellness businesses both in Michigan and Florida.  His extensive Senior Services industry experience compliments his extensive experience in lifestyle coaching, leadership development and innovative program creation and execution.  Steve currently sits on two non-profit boards and has had management responsibility of teams as large as 1000 with gross revenues of $50M+ and as small as two-person start- ups.  Leading innovation discernment and execution as well as talent acquisition, placement and coaching individuals through transformation are common threads throughout Steve’s career.  Steve’s Bachelor of Science education in Exercise Science took place at Eastern Michigan University and Spring Arbor College.

She has held multiple roles and is currently the Administrator for Baptist Home Health Care which is the product of several mergers between hospital based and VNA agencies. She is a board certified from ANCC as Nurse Executive-Advanced. She's a member of Sigma Theta Tau National Honor Society, First Coast Nurse Executives, North East Florida Coalition for Coordinated Care for Seniors, Membership Chair for United Way’s Women In Local Leadership. She is a recipient of North East Florida Great 100 Nurses Award. She had an article published in the Remington Report on Developing a Disease Management Program in 2003. Recently she was very active in programs to decrease rehospializations and improve transitions of care throughout the Baptist Health System. These programs include the creation of Health Connections, a home based primary care service and a very successful telemonitoring program. Her passion is to increase coordination of care to prevent duplication or omission of care.

She possesses extensive knowledge of the WOCN Society best practices relating to in-patient and home based wound management, utilizing this knowledge to develop strategies necessary to ensure the highest possible patient outcomes while maintaining clinical and regulatory compliance. Ms. Labson is a veteran health care professional who has specialized in the provision of home care services since 1977 from both multi-operational and academic perspectives. She has extensive knowledge in the legal, regulatory, and accreditation requirements for the scope of home care programs provided in the United States and Puerto Rico. She is an experienced lecturer, educator, a published author, and functionally fluent in Spanish.

With a B.S. in Health Promotion, Sarah has over 12 years of experience working within the Health and wellness arena.  Her experience stems from serving as a Wellness Consultant and Coach for organizations across the U.S.  Through the creation and establishment of preventative health programs and benefit offerings, Sarah has partnered with numerous employers—including LogMeIn, Inc., Nike, United States Special Operations Command, and Greenville Health System--to increase employee wellness and satisfaction beyond compensation.
